Online appointment requests submitted through our contact form are not confirmed until you receive confirmation from our office via phone, email, or text message. Appointment availability is subject to change based on our office hours.

8.3 Cancellation Policy
We require 24 hours advance notice for appointment cancellations or rescheduling. Failure to provide adequate notice may result in a cancellation fee. This policy allows us to offer your appointment time to other patients who need care.
8.4 No-Show Policy
Patients who fail to show up for scheduled appointments without prior notice may be charged a no-show fee. Repeated no-shows may result in dismissal from the practice.
8.5 Late Arrival
Please arrive 10-15 minutes early for your first appointment to complete necessary paperwork. If you arrive more than 15 minutes late for your appointment, we may need to reschedule you to ensure quality care for all patients.
9. Payment Terms
9.1 Fees and Payment
Payment for services is due at the time of service unless other arrangements have been made in advance. We accept cash, checks, credit cards, and debit cards.
9.2 Insurance
We accept most major insurance plans. However, it is your responsibility to verify that we are in-network with your insurance provider and to understand your coverage, deductibles, co-pays, and any limitations. We will bill your insurance as a courtesy, but you are ultimately responsible for all charges incurred.
9.3 Outstanding Balances
Accounts with outstanding balances for more than 90 days may be subject to collection proceedings. You are responsible for all costs of collection, including reasonable attorney fees and collection agency fees.
